If the 4th string is reading low E, it may be the string is tuned too big and needs to be tuned down until it reaches the right pitch. The same process for the other strings: tune up or down until right pitch is reached.

The other possibility is the quality of the tuner. If you paid a small amount for it, it may be worth spending a little more for a better quality tuner.
